First Time at Altitude: Reading Your Body in Ladakh

Ladakh's beauty is inseparable from its altitude, and the altitude is the one thing first-timers reliably underestimate. It has nothing to do with fitness — marathoners get hit while their less-athletic friends feel fine. Understanding it beforehand is the difference between a trip that unfolds well and one that goes sideways on day two.

A short, plain guide to the thing everyone should read before they go. (This is general information, not medical advice — talk to a doctor about your own situation.)

What acute mountain sickness actually is

As you go higher, there's less oxygen in each breath, and your body needs time to adjust. Push too high too fast and you can get acute mountain sickness (AMS): headache, nausea, dizziness, poor sleep, breathlessness, loss of appetite. Mild AMS is common and usually manageable. What matters is recognising it early and not climbing higher while you have it.

The rules that keep it mild

Rest properly when you first arrive — don't fly into Leh and immediately head for a high pass. Hydrate constantly; it's the simplest and most effective thing you can do. Skip alcohol and heavy exertion in the first days. "Climb high, sleep low" where you can. And ascend gradually rather than in one big jump. Boring advice, but it's what works.

On Diamox and medication

Many travellers discuss Diamox for acclimatisation, but whether it's right for you is a conversation for you and a doctor before the trip — not something to decide from an article. Carry your regular medication, and any the doctor recommends, in easy reach.

When to stop being stubborn

Mild symptoms that ease with rest and water are usually fine. Symptoms that get worse, or don't improve, are a signal to descend — not to push on. The dangerous forms of altitude illness are rare but serious, and the response is always the same: go down, and get help. The mountain will still be there next year; the smart riders are the ones who listen to their bodies rather than their egos.
